Glossopharyngeal neuralgia is recurrent, brief, severe pain in the distribution of the glossopharyngeal nerve (throat, tongue base, tonsillar fossa), triggered by swallowing, talking, or coughing. It may be idiopathic (vascular compression) or secondary. Diagnosis is clinical. Treatment: carbamazepine/oxcarbazepine first-line; microvascular decompression or rhizotomy for medication-refractory cases; rule out secondary causes.
Brief severe throat/tongue-base pain; triggered by swallowing/talking; unilateral; may syncope
Vascular compression or secondary lesion causes glossopharyngeal-nerve ephaptic discharge, producing paroxysmal pain.
Carbamazepine/oxcarbazepine; microvascular decompression/rhizotomy if refractory; rule out secondary causes
Medication adherence; surgical evaluation if refractory.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
