The pathology of disease

Intercostal neuralgia is unilateral pain along the course of an intercostal nerve (chest wall, radiating around the rib), often with tenderness at the nerve exit points. Causes: herpes zoster/postherpetic, nerve compression/trauma, and diabetic neuropathy. Treatment: simple analgesics, neuropathic pain agents (gabapentinoids), and intercostal nerve block; investigate and treat underlying cause (zoster/tumor).

Common Symptoms

Unilateral chest-wall pain; radiates around the rib; tenderness; dermatomal distribution

Causes and Pathogenesis

Zoster/compression/trauma/diabetes irritates the intercostal nerve, producing neuropathic chest-wall pain.

Treatment

Analgesics; gabapentinoids; intercostal nerve block; treat underlying cause

Preventive Measures

Shingles vaccine; treat diabetes; early antiviral for zoster.
