Myasthenia gravis is an autoimmune disorder of the postsynaptic neuromuscular junction (acetylcholine-receptor or MuSK antibodies), characterized by fluctuating weakness and fatigability, worse with activity and improving with rest. Common: ptosis/diplopia (ocular), bulbar (dysarthria/dysphagia), and generalized limb weakness. Myasthenic crisis (respiratory weakness) is an emergency. Treatment: cholinesterase inhibitors (pyridostigmine), immunosuppression (steroids/azathioprine), thymectomy in AChR-positive generalized disease, and IVIG/plasmapheresis for crisis.
Fluctuating weakness/fatigability; ptosis/diplopia; dysarthria/dysphagia; worse with activity; improves with rest
Anti-AChR/MuSK antibodies attack postsynaptic neuromuscular junction, reducing neuromuscular transmission and causing fatigable weakness.
Pyridostigmine; immunosuppression; thymectomy; IVIG/plasmapheresis for crisis; ICU ventilation
Medication adherence; avoid exacerbating drugs; monitor respiratory function.
It is recommended to visit the Neurology department; an appointment can be booked on this platform before the visit.
