Periodic Paralysis

神经内科

Common symptoms

无力、疲劳、四肢对称性软瘫、紧张、醉酒、驻站医、呼吸困难、轻瘫、肾间质受损、感觉障碍

The pathology of disease

Periodic paralysis is a group of inherited channelopathies causing recurrent episodes of acute flaccid weakness, associated with abnormal serum potassium (hypokalemic or hyperkalemic). Attacks are triggered by exercise, high-carbohydrate meals, or potassium intake, lasting hours to days, with preserved consciousness. Treatment: acute potassium correction (hypokalemic) or glucose/insulin (hyperkalemic); long-term by low-carb diet, potassium-sparing diuretics, avoiding triggers (exercise/carb loads); treat thyrotoxic periodic paralysis by controlling hyperthyroidism.

Common Symptoms

Recurrent acute flaccid weakness; attacks hours-days; triggered by exercise/carb/potassium; abnormal serum K

Causes and Pathogenesis

Sodium/calcium-channel mutations cause abnormal muscle-membrane excitability and episodic weakness with K shifts.

Treatment

Acute K correction; long-term low-carb diet, acetazolamide/spironolactone; avoid triggers; treat hyperthyroidism

Preventive Measures

Low-carb diet; avoid strenuous exercise; treat hyperthyroidism; acetazolamide.
