Pulmonary encephalopathy is cerebral dysfunction (headache, confusion, somnolence, coma) due to chronic respiratory failure with hypercapnia/hypoxia (COPD/obesity hypoventilation/neuromuscular disease). Clinical: headache, confusion, slurred speech, papilledema, and somnolence worse in the morning. Treatment: improve ventilation (oxygen/noninvasive ventilation/noninvasive positive-pressure), treat underlying lung disease, correct CO2 retention; avoid excessive oxygen in COPD.
Headache; confusion/somnolence; slurred speech; papilledema; history of chronic respiratory failure
Hypercapnia/hypoxia from chronic respiratory failure causes cerebral vasodilation/dysfunction and encephalopathy.
Improve ventilation (NIV); treat lung disease; correct CO2 retention; cautious oxygen in COPD
Treat COPD; NIV adherence; smoking cessation.
It is recommended to visit the Respiratory department; an appointment can be booked on this platform before the visit.
