Hypertensive encephalopathy is acute cerebral dysfunction due to severe hypertension (typically >180/120 mmHg), presenting with headache, confusion, seizures, visual disturbance, and (in severe) coma, often with posterior leukoencephalopathy on imaging. It is a hypertensive emergency. Treatment: ICU; gradual but urgent BP reduction (IV labetalol/nicardipine, avoid rapid overshoot); control seizures; manage cerebral edema; identify/treat underlying cause; monitor end-organ damage.
Headache; confusion/seizure; visual disturbance; severe HTN; posterior leukoencephalopathy
Severe hypertension causes breakthrough cerebral autoregulation, vasogenic edema, and encephalopathy.
ICU; gradual IV BP reduction; control seizures; manage edema; treat cause
Strict chronic BP control; medication adherence.
It is recommended to visit the Neurology-Emergency-ICU department; an appointment can be booked on this platform before the visit.
