Sciatica is radiating pain along the sciatic nerve distribution (lower back → buttock → posterior/lateral leg → foot). Most commonly due to L5/S1 nerve root compression from lumbar disc herniation; also spinal stenosis, spondylolisthesis, piriformis syndrome, pregnancy. Clinical: unilateral leg pain, numbness, weakness, positive straight-leg raise. Diagnosis: lumbar MRI/X-ray/CT. Treatment: conservative: NSAIDs, gabapentin/pregabalin, muscle relaxants, PT (McKenzie/core strengthening), epidural steroid injections, activity modification; surgical: discectomy/decompression for refractory pain, progressive weakness, or cauda equina; follow-up orthopedics/neurosurgery/neurology/PT.
Radiating leg pain; numbness; weakness; positive SLR
Lumbar disc herniation/spinal stenosis/other causes compress L5/S1 nerve root.
NSAIDs/PT/epidural; discectomy if refractory
