Purulent renal cortical infection is a severe staphylococcal infection reaching the cortex hematogenously. An unliquefied inflammatory mass is called acute focal bacterial nephritis; when abscesses form it is a cortical abscess or suppurative nephritis, and fused abscesses form a renal carbuncle. With timely antibiotics the incidence has declined, mostly presenting as acute focal bacterial nephritis.

The commonest pathogen is S. aureus, reaching the kidney via blood from other pyogenic foci such as furuncles, carbuncles, abscesses, infected wounds, upper respiratory infection or adjacent tissue infection; occasionally secondary to obstruction or congenital anomalies such as childhood VUR. Early lesions are multiple small cortical abscesses that may fuse into multilocular abscesses or a renal carbuncle; rarely they rupture the capsule into perinephric fat.
Antibiotics, supportive treatment
Promptly treat skin infection foci to prevent hematogenous spread.
It is recommended to visit the nephrology department; before the visit you may first book an appointment and connect with experts through this platform.
