Perinephritis is a bacterial infection between the renal capsule and perinephric fascia; if it forms an abscess it is a perinephric abscess. It mostly spreads directly from pyelonephritis, usually gram-negative bacilli, especially E. coli. A minority spreads hematogenously from other foci to the cortex forming small abscesses that rupture into the perinephric space; these are mostly gram-positive cocci. Typical features are chills, high fever and renal pain; spread to adjacent organs causes their infection.

Perinephritis/perinephric abscess may come from many pathogens; with widespread broad-spectrum antibiotics, hematogenous infection has declined, shifting from S. aureus to E. coli as the main pathogen.
