Diabetes Insipidus

肾内科

Common symptoms

失眠、经常口渴、肾性尿崩、驻站医、肾上腺皮质激素不足、多饮、口渴、多尿、尿崩

The pathology of disease

Diabetes insipidus (DI) is passage of large volumes of dilute urine with thirst/polydipsia. Central (cranial) DI = ADH (vasopressin) deficiency from pituitary-hypothalamic disease (tumor, trauma, surgery, infiltrative); nephrogenic DI = renal collecting-duct resistance to ADH (hereditary V2 receptor/aquaporin-2 mutation, drugs such as lithium, hypercalcemia, hypokalemia). Water-deprivation test distinguishes; central DI responds to desmopressin.

Common Symptoms

Polyuria (3-20 L/day); polydipsia; dilute urine; nocturia; dehydration if thirst impaired; hypernatremia

Causes and Pathogenesis

Central: ADH deficiency causes impaired water reabsorption. Nephrogenic: renal collecting duct fails to respond to ADH, producing dilute urine regardless of ADH.

Treatment

Central: desmopressin (DDAVP); nephrogenic: treat cause, thiazides, amiloride, low-solute diet; ensure free water access

Preventive Measures

Monitor urine output after pituitary surgery/trauma; avoid lithium where possible; ensure water access.

Department to Visit
