Hypomagnesemia is serum magnesium below ~0.7 mmol/L. Clinical: neuromuscular irritability (tetany, Chvostek/Trousseau signs, tremor, seizures), arrhythmias (torsades), and refractory hypokalemia/hypocalcemia. Causes: diuretics, GI loss (diarrhea/fistula), alcoholism, malnutrition, renal magnesium-wasting, and proton-pump inhibitors. Treatment: magnesium repletion (oral/IV), treat underlying cause.
Tetany; tremor; seizures; arrhythmia; weakness; refractory hypokalemia/hypocalcemia
GI/renal magnesium loss or poor intake lowers serum Mg, impairing neuromuscular/excitability and PTH release, causing concurrent hypokalemia/hypocalcemia.
Oral/IV magnesium repletion; treat underlying cause; monitor ECG/electrolytes
Monitor Mg on diuretics/PPIs; treat alcoholism; replete in refeeding.
It is recommended to visit the Internal Medicine department; an appointment can be booked on this platform before the visit.
