Mast cell leukemia (MCL, tissue basophil leukemia) is a clonal mast cell malignancy; transformation from systemic mastocytosis II/III or de novo. Two categories: (1) infiltration: hepatosplenomegaly/lymphadenopathy/bone lesions/urticaria pigmentosa/skin wheal; (2) cytoplasmic granules releasing heparin/hyaluronic acid/histamine/glycosaminoglycans/kinins/prostaglandins → refractory peptic ulcer, abdominal pain, hematemesis, melena, headache, edema.
Macule; bleeding tendency; lymphadenopathy; dyspnea; melena; abdominal pain; fatigue
Possibly c-KIT mutation. AML-like pathology with mast cell predominance; urticaria pigmentosa; hyperhistaminemia → flushing/pruritus/bronchospasm/hypotension/shock; peptic ulcer/bleeding/perforation; heparin-induced bleeding.
Drug therapy; supportive care
No prevention; early detection/treatment.
