血液科

Common symptoms

腹腔积液、肝脾肿大、表情淡漠、胸腔积液、腹水、乏力

The pathology of disease

Adult T-cell leukemia (ATL) is a special type of adult lymphoid malignant clonal proliferation directly related to HTLV-I infection; lesions mainly in peripheral lymphocytes; marrow may also be involved.

Common Symptoms

Ascites; hepatosplenomegaly; apathy; pleural effusion; ascites; fatigue

Causes and Pathogenesis

(1) Etiology: HTLV-I infection; serum positive; endemic in southern Kyushu, Japan (10-15% antibody-positive); familial clustering (3-4x). (2) Mechanism: long latency. (a) Tax protein: LTR U3/R/U5; Tax transactivates via ATF/CREB and NF-kappaB/C-Rel; activates IL-2/IL-2Ralpha, c-fos, c-jun, PTHrP → T-cell autocrine growth, hypercalcemia; accelerates G1→S; NF-kappaB activation. (b) Immunosuppression: TGF-beta1 upregulation; new HLA-I/II epitopes. (c) Oncogene activation/tumor-suppressor inactivation (c-fos transactivation).

Treatment

Drug, supportive therapy

Preventive Measures

Wash/peel fruit (pesticide residue); keep food/utensils clean.
