Warm-Antibody Autoimmune Hemolytic Anemia

血液科

Common symptoms

寒战、昏迷、休克、背痛、腹泻、头昏、免疫性溶血

The pathology of disease

Warm-antibody autoimmune hemolytic anemia: autoantibodies optimally react with RBCs at 35-40C (warm antibodies). They are divided into incomplete warm antibodies and warm autoimmune hemolysins. By chemical structure, warm antibodies are IgG, IgM or IgA; IgG warm antibodies are further divided into IgG1, IgG2, IgG3 and IgG4 subtypes.

Common Symptoms

Anemia; jaundice; splenomegaly; dark urine

Causes and Pathogenesis

IgG warm antibodies bind RBCs at body temperature; splenic macrophages phagocytose antibody-coated RBCs (extravascular hemolysis); complement activation may occur. Primary or secondary to CLL, lymphoma, SLE, drugs (alpha-methyldopa). Direct antiglobulin test (DAT/Coombs) positive.

Treatment

Corticosteroids; IVIG; splenectomy; immunosuppressants; rituximab

Preventive Measures

Treat underlying disease; avoid offending drugs; monitor.
