血液科

Common symptoms

不明原因发热、血红蛋白尿、遗传性出血性毛...

The pathology of disease

Microangiopathic hemolytic anemia is a severe diffuse thrombotic microangiopathy characterized by microangiopathic hemolytic anemia, consumptive thrombocytopenia from platelet aggregation, and organ damage (kidney, central nervous system) caused by microthrombi. It is a hemolytic-anemia syndrome in which RBCs are fragmented by diseased microvessels.

Common Symptoms

Hemolytic anemia; thrombocytopenia; renal failure; neurologic symptoms; fever

Causes and Pathogenesis

Seen in hemolytic-uremic syndrome (HUS), thrombotic thrombocytopenic purpura (TTP), DIC, malignant hypertension, scleroderma, cancer. Fibrin/platelet thrombi in microvessels mechanically shear RBCs producing schistocytes.

Treatment

Plasma exchange; dialysis; supportive care; treat underlying cause

Preventive Measures

Prompt recognition; plasma exchange is life-saving in TTP.
