Paroxysmal cold hemoglobinuria (PCH) is a rare disease characterized by sudden hemoglobinuria after systemic or local cold exposure. Hemolysis is usually triggered by cold, but triggers may be subtle. Onset occurs minutes to hours later with brief chills and fever.
Dark urine after cold exposure; chills; fever; abdominal pain; anemia
Donath-Landsteiner antibody (IgG) binds RBCs at low temperature and fixes complement; on rewarming, complement-mediated intravascular hemolysis occurs. Often post-infectious (syphilis, viral) in children.
Avoid cold; supportive care; treat underlying infection
