Idiopathic thrombocytopenic purpura (ITP) is an autoimmune disease in which anti-platelet antibodies cause platelet destruction and thrombocytopenia. The acute form is common in children (post-viral, self-limited); the chronic form is common in adults (women 2:1). Clinical features: skin petechiae/ecchymoses, gingival/nasal bleeding, menorrhagia; severe cases have visceral/intracranial bleeding.
Skin petechiae/ecchymoses; gingival/nasal bleeding; menorrhagia; fatigue; severe: visceral/intracranial bleeding
Autoantibodies (usually IgG) bind platelet glycoproteins (GPIIb/IIIa, GPIb/IX); splenic macrophages destroy antibody-coated platelets; marrow megakaryocytes are normal/increased but cannot compensate.
Corticosteroids; IVIG; splenectomy; rituximab; thrombopoietin-receptor agonists; platelet transfusion only for life-threatening bleeding
Avoid aspirin/NSAIDs/trauma; treat promptly; monitor platelet count.
It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
