Sulfhemoglobinemia is the presence of sulfhemoglobin (a sulfur-bound, non-functional hemoglobin derivative) in circulating blood. It is usually caused by drugs (sulfonamides, dapsone, nitrates/nitrites) or severe constipation; it causes cyanosis that does not respond to methylene blue. The abnormal RBC must be degraded normally; no specific antidote, withdraw the offending drug.
Cyanosis; slate-gray skin; dark blood; dyspnea; headache
Oxidant drugs or hydrogen sulfide from gut convert hemoglobin to sulfhemoglobin, which cannot carry oxygen and is not reducible; persists for RBC lifespan.
Withdraw offending drug; supportive care; exchange transfusion in severe cases
Award oxidant/sulfonamide drugs; treat constipation.
It is recommended to visit the Hematology department; an appointment can be booked on this platform before the visit.
