Toxic (acquired) methemoglobinemia is methemoglobin elevation caused by oxidant drugs/chemicals (nitrites, aniline dyes, dapsone, benzocaine, nitroglycerin) that oxidize hemoglobin iron from Fe2+ to Fe3+, producing methemoglobin that cannot carry oxygen. Clinical: cyanosis, chocolate-brown blood, dyspnea, headache; severe (>50%) causes hypoxia, seizures, arrhythmia.
Cyanosis; chocolate-brown blood; dyspnea; headache; dizziness; anxiety; severe: seizures/arrhythmia
Oxidant chemicals overwhelm the NADH-methemoglobin-reductase reducing system, accumulating Fe3+ hemoglobin; left-shift of oxygen-dissociation curve worsens tissue hypoxia.
Remove exposure; methylene blue (1-2 mg/kg IV); ascorbic acid; oxygen; exchange transfusion if severe
Avoid oxidant drugs/chemicals; monitor at-risk workers; caution in G6PD deficiency (methylene blue may worsen hemolysis).
It is recommended to visit the Emergency department; an appointment can be booked on this platform before the visit.
