Familial Mediterranean fever (FMF) is an autosomal-recessive MEFV (pyrin) mutation causing recurrent self-limited attacks of fever with serositis: peritonitis (acute abdomen), pleuritis, synovitis, and erysipelas-like erythema. It affects Mediterranean populations (Armenians, Sephardic Jews, Arabs). Long-standing disease causes secondary (AA) amyloidosis with renal failure. Treatment: colchicine prevents attacks and amyloidosis.
Recurrent fever; abdominal/chest pain (serositis); arthritis; erysipelas-like leg erythema; attack duration 1-3 days
MEFV pyrin mutation causes dysregulated IL-1-mediated inflammation, triggering recurrent serosal inflammation and amyloid deposition.
Colchicine prophylaxis; IL-1 inhibitors (anakinra/canakinumab) for colchicine-resistant; treat amyloidosis
Lifelong colchicine; monitor renal/amyloid; genetic counseling.
It is recommended to visit the Rheumatology department; an appointment can be booked on this platform before the visit.
