不明原因发热、驻站医、乏力
Rheumatic fever is a delayed autoimmune complication of group A streptococcal pharyngitis, occurring in children, presenting with Jones-criteria: migratory polyarthritis, carditis (pancarditis), Sydenham chorea, erythema marginatum, and subcutaneous nodules, plus evidence of preceding strep (elevated ASO). It causes chronic rheumatic valvular disease. Treatment: eradicate streptococcus (penicillin); anti-inflammatory (aspirin/steroids for carditis); long-term secondary penicillin prophylaxis to prevent recurrence and valvular damage.
Migratory polyarthritis; carditis; chorea; erythema marginatum; subcutaneous nodules; child after strep pharyngitis; elevated ASO
Antibodies to group A strep cross-react with cardiac/joint/brain tissue (molecular mimicry).
Penicillin to eradicate strep; aspirin/steroids for inflammation; long-term secondary penicillin prophylaxis
Treat strep pharyngitis promptly; secondary prophylaxis after RF.
It is recommended to visit the Pediatrics/Cardiology/Rheumatology department; an appointment can be booked on this platform before the visit.
