Inclusion-body myositis is the most common acquired myopathy in older adults, characterized by slowly progressive, asymmetric weakness of distal fingers and quadriceps, with muscle biopsy showing rimmed vacuoles/inclusion bodies. It does not respond well to immunosuppression. Treatment: supportive care, physical/occupational therapy, and falls prevention; immunosuppression is generally ineffective; some trialing of IVIG.
Slowly progressive asymmetric weakness; finger flexors/quadriceps; older adults; falls; no good response to steroids
Debate: degenerative/autoimmune; rimmed-vacuole inclusion-body myofibers cause progressive weakness.
Supportive; physical/occupational therapy; falls prevention; immunosuppression generally ineffective
