Iliac-vein compression syndrome (May-Thurner syndrome) is compression of the left common iliac vein between the right common iliac artery and the lumbar spine, causing left leg swelling/pain and deep-vein thrombosis in young/middle-aged patients. Treatment: anticoagulation; catheter-directed thrombolysis for acute DVT; iliac-vein angioplasty/stenting; surgical decompression; compression stockings; long-term follow-up; screen for hypercoagulability.

Common Symptoms

Left leg swelling/pain; DVT; young/middle-aged; left-sided

Causes and Pathogenesis

Right CIA compresses left common iliac vein against spine, causing stasis, spurs, and DVT.

Preventive Measures

Early diagnosis; treat compression before extensive DVT.
