Paralytic Ileus

Paralytic ileus is functional obstruction due to impaired bowel motility (postoperative/infection/electrolyte imbalance/medication/spinal injury), presenting with abdominal distension, vomiting, and absent bowel sounds. Treatment: NPO; nasogastric decompression; IV fluids/electrolyte correction (especially potassium); treat cause (infection/medication/electrolyte); ambulation; chewing gum (post-op); prokinetics in selected; avoid opioids; surgical exploration if mechanical obstruction suspected; monitor for ischemia; follow-up.

Causes and Pathogenesis

Impaired bowel motility causes functional obstruction.

Treatment

NPO; NG decompression; correct electrolytes; treat cause; ambulation; avoid opioids
