Volvulus

Volvulus is twisting of bowel on its mesentery (sigmoid/cecal common), causing closed-loop obstruction and ischemia, presenting with acute abdominal pain, distension, and vomiting. Treatment: NPO; IV fluids/electrolytes; urgent endoscopic decompression (rigid sigmoidoscopy for sigmoid) if non-perforated; surgical detorsion/resection if endoscopic fails/perforated/necrosis; bowel resection if ischemic; antibiotics; monitor for ischemia/recurrence; follow-up; early surgery reduces mortality.
