Cecal volvulus is twisting of cecum (due to anomalous fixation), causing closed-loop obstruction and ischemia, presenting with acute right-sided abdominal pain and distension. Treatment: NPO; IV fluids; urgent surgical exploration; detorsion + cecopexy or resection (right hemicolectomy) if necrotic/perforated; endoscopic decompression rarely successful; antibiotics; monitor for ischemia/recurrence; follow-up.
