Retroperitoneal fibrosis (RPF) was first reported by the French urologist Albarran in 1905, and after Ormond reported 2 cases in 1948 it became more widely recognized. Pathologically it is characterized by proliferation of retroperitoneal fibrous tissue causing diffuse fibrosis; clinical features closely relate to the degree of compression of retroperitoneal structures or organs such as the ureter.

About 2/3 have unknown cause, called idiopathic retroperitoneal fibrosis; the other 1/3 may relate to drugs, tumors, trauma or surgery, bleeding, urine extravasation, radiation, nonspecific GI inflammation (such as Crohn disease), appendicitis and diverticulitis.
