Gas Gangrene

普外科

Common symptoms

坏死性筋膜炎、虚脱、粒细胞减少、指趾端干性坏死、面色苍白、候亚益、剧痛、神志淡漠、下肢湿性坏疽、低热

The pathology of disease

Gas gangrene (clostridial myonecrosis) is a severe clostridial exotoxin infection characterized by myonecrosis and systemic toxicity, abrupt onset and rapid progression. C. perfringens causes ~80%; others include C. septicum, C. novyi, C. histolyticum, C. bifermentans, C. sporogenes. More than one Clostridium species or other bacteria may co-infect; organisms come from soil or endogenous gut/biliary sources. At least 12 exotoxins exist; 7 kill mice on intraperitoneal injection.

Common Symptoms

Necrotizing fasciitis; collapse; granulocytopenia; dry distal necrosis; pallor; Hou Yayi; severe pain; apathy; wet lower-limb gangrene; low fever

Causes and Pathogenesis

Clostridia are Gram-positive anaerobic rods (C. perfringens, C. oedematiens, C. septicum, C. sporogenes, C. histolyticum); clinical cases are often mixed. Occurrence depends not merely on clostridia but on host resistance and wound conditions—an anaerobic environment is required. Clostridia are ubiquitous in soil/feces but may not cause disease. Risk factors: dehydration, blood loss, shock, extensive tissue necrosis, deep muscle damage (thigh/buttock), retained fragments, open fractures with major vessel injury, prolonged tourniquet. Pathogenesis: pathogens multiply mainly in wounds (rarely causing septicemia). C. perfringens produces α-toxin, collagenase, hyaluronidase, fibrinolysin, DNase; RBC destruction causes hemolysis/hemoglobinuria/oliguria/renal necrosis; extensive muscle necrosis spreads rapidly. Carbohydrate fermentation produces gas (tissue swelling); protein/gelatin decomposition produces H2S (foul odor). Local ischemia, plasma exudation, and toxins worsen necrosis, favoring bacterial growth. Massive necrosis and exotoxin absorption cause severe toxemia; some toxins directly damage heart/liver/kidney with focal necrosis.

Treatment

Drug therapy, rehabilitation

Preventive Measures

Thorough debridement is the most reliable prevention; debridement within 6 h almost completely prevents gas gangrene. Even after 6 h, antibiotics + debridement help. Open war wounds after debridement. Suspected wounds are irrigated with 3% H2O2 or 1:1000 KMnO4; remove sutures. Penicillin/tetracycline help but do not replace debridement. Isolate patients; boil clothes/dressings ≥1 h or autoclave; burn dressings.

Department to Visit

It is recommended to visit the General Surgery department; an appointment can be booked on this platform before the visit.
