Gastric lipoma is a benign gastric mesenchymal tumor; low incidence, slow progression, rare malignant change, good prognosis. Middle-aged; no gender difference.
Dull gastric pain; upper abdominal discomfort; GI bleeding
(1) Etiology: fat develops at fetal 14 weeks; lobules by 24 weeks; 4 stages: primitive mesenchymal → preadipocyte → lipoblast → mature adipocyte; lipomas mostly mature. (2) Mechanism: body/antrum (antral common); 90% submucosal (intraluminal); 10% subserosal (exogastric). Gross: 80-90% solitary; 10-20% multiple; sessile spherical/lobulated; 2-5cm (up to 15cm); smooth encapsulated, soft, mobile in wall; mucosal erosion/ulcer from friction; yellow semi-transparent cut surface. Microscopy: mature adipocytes, fibrous trabeculae; variants: fibrolipoma/angiolipoma/myxolipoma. Infiltrating gastric lipoma: fat cells between smooth muscle fibers, no lobules/atypia; distinguish from liposarcoma.
