Renal-artery stenosis is narrowing of the renal artery (most often atherosclerotic, or fibromuscular dysplasia), causing renovascular hypertension and ischemic nephropathy. Clinical: resistant hypertension, flash pulmonary edema, worsening renal function with ACEi/ARB. Treatment: medical therapy (ACEi/ARB with monitoring, antiplatelet/statin, BP control); renal artery angioplasty/stenting for significant stenosis with refractory HTN/recurrent flash edema/declining renal function; treat underlying cause (FMD - PTA; atherosclerosis - risk-factor control).
Resistant HTN; flash pulmonary edema; worsening renal function; by atherosclerotic/FMD
Atherosclerosis/FMD narrows the renal artery, causing renin-mediated HTN and ischemic nephropathy.
ACEi/ARB with monitoring; antiplatelet/statin; PTA/stenting for significant symptomatic stenosis; treat cause
Control atherosclerosis risk factors; screen young women with HTN for FMD.
It is recommended to visit the Nephrology/Vascular-surgery department; an appointment can be booked on this platform before the visit.
