Vesicoureteral reflux means congenital or acquired loss of the anti-reflux function of the intravesical ureter; when urine accumulates or the detrusor contracts and intravesical pressure rises, urine flows back from the bladder into the ureter and even pelvis. Causes include congenital incompetence of the vesicoureteral valve or secondary obstruction and neurogenic bladder.
Secondary hypertension; chronic renal insufficiency; frequency; pyuria; fatigue; nausea; secondary infection; dysuria; urgency
Anatomic ureteral defect (35%): the ureter shifts laterally and the submucosal segment shortens, losing anti-reflux ability; normal submucosal ureteral length:diameter is 5:1, but in reflux only 1.4:1. Raised intravesical pressure (25%): from lower obstruction (urethral stricture, late prostatic hyperplasia) or neurogenic bladder. Abnormal ureteral orifice (18%): also a cause of reflux.
Antibiotics, surgery, supportive treatment
Prevent and treat urinary tract infection promptly; treat underlying obstruction and malformation.
