Perinephric abscess

泌尿外科

Common symptoms

腰大肌阴影消失、脓尿、胸腔积液、测试、肺不张

The pathology of disease

A perinephric abscess mainly forms when an intrarenal abscess ruptures into the perinephric space, so pathogens are the same; about 25% grow multiple pathogens. Perinephric fascia usually localizes it. Unlike acute pyelonephritis, symptoms precede admission for longer and fever persists despite antibiotics; symptoms last over 5 days versus under 5 days in acute pyelonephritis.

Common Symptoms

Loss of psoas shadow; pyuria; pleural effusion; test; atelectasis

Causes

Mainly rupture of an intrarenal abscess into the perinephric space. With widespread broad-spectrum antibiotics, hematogenous infection has declined; pathogens shifted from S. aureus to E. coli and Proteus, then S. aureus, plus many gram-negative bacilli (Klebsiella, Enterobacter, Pseudomonas, P. aeruginosa); anaerobes such as Clostridium and Actinomyces can also cause it.

Treatment

Drainage, antibiotics

Prevention

Treat intrarenal infection promptly to prevent rupture.
