Granulomatous prostatitis

Granulomatous prostatitis is a rare, mostly nonspecific disease often related to recent urinary infection; on digital rectal exam the prostate feels hard or diffusely nodular, hard to distinguish from prostate cancer.

Common Symptoms

Yan Tie; frequency; granuloma; hematuria; nodule; urethral burning; dysuria

Causes

It relates to a strong local foreign-body reaction; prostatic duct obstruction is primary, from bacterial inflammation or surgical trauma causing tissue necrosis; BPH may obstruct ducts; infection and inflammation destroy duct/gland epithelium.

Treatment

Antibiotics, observation, biopsy to exclude cancer

Prevention

Treat urinary infection promptly.
