Epididymitis

Epididymitis is common in young/middle-aged men; when resistance is low, E. coli, staph and strep ascend the vas deferens to infect the epididymis. It is acute or chronic. Acute epididymitis usually spreads from urinary infection, prostatitis and seminal vesiculitis along the vas; hematogenous spread is rare; urethral instrumentation, frequent catheterization and post-prostatectomy catheters predispose.

Common Symptoms

Lower abdominal dull ache; scrotal dragging pain; thickened vas; groin pain; scrotal swelling; inguinal lymphadenopathy; Dai Chao; dragging sensation; scrotal enlargement; distending pain

Causes

Acute epididymitis spreads from urinary infection, prostatitis and seminal vesiculitis along the vas deferens.

Treatment

Antibiotics, scrotal support, rest

Prevention

Treat urinary/prostate infection promptly, avoid instrumentation trauma.
