Priapism

Priapism is persistent erection unrelated to libido; erection over 6 hours is abnormal. Traditionally primary (idiopathic) or secondary; by hemodynamics low-flow (ischemic) or high-flow (non-ischemic); it is also acute, intermittent (recurrent, e.g. sickle cell anemia) or chronic (usually high-flow). It begins as physiologic erection then progresses to high-flow type.

Common Symptoms

Dyschezia; abnormal penis

Causes

It occurs in spinal stenosis, spinal cord injury and disc herniation; mechanism may be parasympathetic-induced release of erectile neurotransmitters; also sickle cell disease, leukemia and drugs.

Treatment

Emergency aspiration/injection, surgical shunt for ischemic type

Prevention

Early treatment of prolonged erection to preserve function.
