Trichomonal urethritis (urethral trichomoniasis) affects both sexes, more common in women. Women acquire T. vaginalis by swimming/bathing/sex; it inhabits the vagina, then causes urethritis, then infects men via sex. Besides urethritis it may cause cystitis/prostatitis.

Three requirements: source, route, susceptible host. Etiology: T. vaginalis is a flagellate protozoan with trophozoite only; pear-shaped 7-32 µm, refractile, with 3-5 anterior flagella (avg 4), undulating membrane, oval nucleus, posterior axostyle. Hardy: lives 21 days at 3-5°C, 20-40 min at 46°C, 6 h in semi-dry conditions—so can transmit outside the body. Reservoir: patients/carriers; mainly sexual, also public baths/pools/toilets. Whether pure trichomonads cause urethritis is debated—often other pathogens (normal flora, C. trachomatis, mycoplasma) coexist. Routes: direct (sex) and indirect (public baths, pool chairs/toilet seats contaminated by carriers, shared underwear/rental swimwear). Pathogenesis: trichomonads consume glycogen in vaginal epithelium, suppress lactobacilli, raise vaginal pH from acidic to neutral/alkaline—destroying self-cleaning. Commercial pH-4 acidic feminine washes help maintain this. In vitro, trichomonads secrete cytotoxic adhesion factors. Incubation: 4-28 days.
Drug therapy, supportive therapy
Eight tips: wash hands; wash vulva then anus (never reverse), personal towels/basins; use pH-4 sanitary products; drink lots (urinate every 2-3 h); bathe separately, change underwear often, cotton breathable underwear, avoid tight jeans; adequate sleep, moderate sex (>3×/week raises UTI risk); urinate after sex. Male: abstain until cured, avoid alcohol/spicy food, isolate household items, learn safe sex, use condoms. Female: drink lots; avoid urinary catheters/instruments when possible; disinfect and culture after use; closed drainage systems reduce catheter-associated UTI.
It is recommended to visit the Urology department; an appointment can be booked on this platform before the visit.
