Perineal pain; dysuria; urethral discharge; tender prostate on exam; STI exposure; fever

Causes

Neisseria gonorrhoeae; ascending STI; rectal/urethral spread.

Treatment

Ceftriaxone IM + azithromycin/doxycycline (also covers chlamydia); treat partner; test for other STIs
