Spinal epidural abscess is a collection of pus in the spinal epidural space, usually from hematogenous spread (skin infection, IV drug use, diabetes) or contiguous spread. Clinical: fever, severe localized back pain, radicular pain, and rapidly progressive myelopathy (weakness, sensory level, bladder/bowel dysfunction). Delay leads to permanent paralysis. Diagnosis: MRI. Treatment: urgent surgical drainage/decompression plus prolonged IV antibiotics.
Fever; severe localized back pain; radicular pain; progressive weakness/myelopathy; bladder/bowel dysfunction; risk factors (diabetes/IVDU)
Hematogenous/contiguous infection forms an epidural pus collection that compresses the spinal cord/roots and causes ischemia.
Urgent surgical drainage/decompression; prolonged IV antibiotics; neurorehab; manage bladder
Treat skin infection; control diabetes; avoid IVDU; sterile spinal procedures.
It is recommended to visit the Neurosurgery department; an appointment can be booked on this platform before the visit.
