Cerebral schistosomiasis: egg granulomas in brain, 2-4% of schistosomiasis; usually from lung lesions. Temporo-occipital cortical/subcortical abscesses/eosinophilic granulomas; acute (meningoencephalitis) vs chronic (seizures/intracranial hypertension).
Seizure; paralysis; nodule; altered consciousness; convulsion; focal signs; ICP elevation; hyperreflexia
Life cycle: adult/egg/miracidium/sporocyst/cercaria/schistosomulum. S. japonicum 1000-3000 eggs/day (10× mansoni/haematobium). Egg granuloma = cell-mediated delayed hypersensitivity (SEA → T cells; Th1 IL-2/IFN-γ; Th2 IL-4/5/10; TNF-2/IL-1). Japanese = clustered eggs, abscess-like liquefaction, PMN/PL-rich granulomas. Acute = mixed immune; chronic = delayed. Brain: frontoparietal/temporal gray-white junction; gliosis/edema; adults not found in cerebral veins.
Drug therapy, supportive therapy
Screen/treat patients/livestock; fecal management; snail control; avoid infested water.
