Rheumatic mitral insufficiency is mitral regurgitation due to rheumatic deformity/retraction of leaflets and chordae, causing left-atrial/ventricular volume overload, dyspnea, and fatigue. Treatment: diuretics/vasodilators (ACE inhibitors) for symptoms; surgical valve repair/replacement when symptomatic or with left-ventricular dysfunction/dilation; secondary penicillin prophylaxis.
Dyspnea/palpitations; systolic murmur; LA/LV enlargement; history of rheumatic fever
Rheumatic retraction/fusion of leaflets/chordae prevents coaptation, causing regurgitation.
Diuretics/ACEi; surgical repair/replacement when symptomatic/LV dysfunction; secondary prophylaxis
