Fasciitis

骨外科

Common symptoms

胸背痛、肌肉紧张性疼痛、肌肉痉挛性疼痛、肌肉酸痛

The pathology of disease

Fasciitis refers to an aseptic inflammatory reaction of muscle and fascia; when the body is stimulated by adverse external factors such as wind-cold invasion, fatigue, trauma or improper sleeping posture, it can trigger an acute attack of myofascitis. Acute or chronic injury, strain and cold exposure of the muscles, ligaments and joint capsules of the shoulder, neck and lower back are the basic causes. In some patients symptoms are relieved by rest; hot compresses and massage can disperse nodules, and local block of painful nodules is quite effective, though lumbar muscle exercise is perhaps most important. A few patients with stubborn, refractory symptoms require surgery; during surgery a fissure in the local fascia may be found with fatty tissue herniating through it, which is the nodule palpated clinically. The fat adheres to surrounding tissue including the fascia and adjacent cutaneous nerve branches, which may be the cause of pain. Surgery should remove the nodule, repair the fascia, divide adhesions and resect the cutaneous nerve. Results are often good, but because the lesion is often multiple, surgery can only relieve symptoms at one site, so surgical indications must be strictly controlled.

Common Symptoms

Chest and back pain; muscle tension pain; muscle spasmodic pain; muscle soreness

Causes

A humid, cold climate is one of the most common causes: damp cold contracts the blood vessels of the back muscles, and ischemia and edema cause local fibrous serous exudation, eventually forming fibrositis. Chronic strain is another important factor: after back muscles and fascia are injured, fibrotic changes leave the soft tissue in a state of high tension, causing minor tearing injuries, which in turn increase and contract fibro-like tissue, compressing local capillaries and peripheral nerves and causing pain. Other triggers include viral infection and muscular allergic reactions in rheumatism. Frequent long walking, including hiking, trekking and shopping, over several days, easily causes chronic injury of the sole, leading to plantar fasciitis. In addition, an overly hard heel presses on the heel and can also cause plantar fasciitis, and wearing high heels often worsens sole injury. Necrotizing fasciitis is a relatively rare severe soft-tissue infection that, unlike streptococcal necrosis, is often a mixed infection of multiple bacteria including Gram-positive hemolytic streptococci, Staphylococcus aureus, Gram-negative bacteria and anaerobes; in the past anaerobes were often missed due to poor culture techniques, but recently anaerobes such as Bacteroides, Peptostreptococcus and cocci have been confirmed as common pathogens, though rarely as a pure anaerobic infection.

Treatment

Surgery, medication, supportive treatment, rehabilitation therapy

Prevention

1. Remove the triggers, maintain hygiene, strengthen exercise to improve immunity and prevent infection. 2. Diagnose and treat early; do not give up treatment easily when the disease is in remission.

Recommended Department

It is recommended to visit the orthopedic surgery department; before the visit you may first book an appointment and connect with experts through this platform.
