Syphilitic Arthritis

骨外科

Common symptoms

疱疹、心力衰竭、发炎、脓疱、剧痛、疥疮、人格改变、角膜炎、继发感染、惊厥

The pathology of disease

Syphilitic arthritis usually occurs at 20-40 years; some have an incubation >10 years before joint symptoms; latent syphilis may be triggered by trauma/delivery/infection. Congenital syphilitic arthritis mainly occurs at 6-10 years, rarely after 20. Syphilitic arthralgia must be distinguished from neuralgia and hysteria; syphilitic osteoarthritis from ordinary osteoarthritis; white-swelling joint syphilis from tuberculous arthritis; edematous joint syphilis from tuberculous synovitis, effusion, chronic rheumatic arthritis, or nonspecific synovitis/effusion.

Common Symptoms

Vesicle; heart failure; inflammation; pustule; severe pain; scabies; personality change; keratitis; secondary infection; convulsion

Causes and Pathogenesis

(1) Etiology: T. pallidum was discovered by Schaudinn and Hoffman (1905); it is closely related to T. p. pertenue (yaws) and T. carateum (pinta). It is a slender spiral organism ~0.15 µm wide × 6-50 µm long with 6-14 coils, tapered ends; too thin for Gram stain, visible by dark-field microscopy/silver/fluorescent antibody. Its outer membrane is unusually phospholipid-rich with few exposed surface proteins—explaining how syphilis progresses despite active antibody responses to internal antigens. Six axial filaments (related to flagella) enable motility. Long-term in vitro culture is difficult; rabbit serial passage maintains virulence; all studied strains are penicillin-sensitive and antigenically similar. Natural hosts are humans, some monkeys, and higher apes. (2) Pathogenesis: T. pallidum penetrates intact mucosa or minor epithelial breaks. In rabbit syphilis, treponemes appear in lymphatics within 30 min and soon in blood; transfusion syphilis shows human disease is systemic from the start. Chancre at the inoculation site reflects the large inoculum there; inoculum size inversely correlates with chancre time. Doubling time ~33 h, explaining chronicity and long incubation. No exotoxin has been found; adhesion is specific but cytotoxicity unproven; most treponemes are intercellular, rarely intracellular.

Treatment

Drug therapy, supportive therapy

Preventive Measures

Control relies on clinical vigilance, reporting, epidemiology, and prophylactic treatment of contacts. Systemic antisyphilitic therapy is required; cure is as difficult as in neurosyphilis—long-term adequate treatment until serology is negative. Mercury preparations were once specific; penicillin + arsphenamine + iodide combinations are effective; surgery is rarely needed.

Department to Visit

It is recommended to visit the Orthopedic Surgery department; an appointment can be booked on this platform before the visit.
