Tubo-ovarian abscess usually develops from acute salpingitis. Inflammation adheres the fimbria and isthmus; inflammatory secretions cannot drain and accumulate as pyosalpinx. Pure ovarian pyocyst is rare and usually also from acute salpingitis. If the fimbria is open during acute salpingitis, purulent discharge spills into the pelvis causing widespread adhesions enclosing tube and ovary, gradually forming a tubo-ovarian abscess.
