Hepatorenal Syndrome

康复科

Common symptoms

肝肾综合征、食欲不振、腹水、氮质血症、乏力、肾小管坏死、少尿、消瘦、厌食、扑翼样震颤

The pathology of disease

Hepatorenal syndrome is functional renal failure in advanced cirrhosis/acute liver failure, due to splanchnic vasodilation and renal vasoconstriction, presenting with oliguria/azotemia. Treatment: exclude prerenal/intrinsic causes; volume expansion with IV albumin; vasoconstrictors (terlipressin/norepinephrine); avoid nephrotoxins (NSAIDs/contrast/aminoglycosides); treat precipitants (infection/bleeding/diarrhea); TIPS in selected; liver transplant is definitive; monitor urine output/creatinine/electrolytes; ICU if severe; follow-up.

Common Symptoms

Oliguria; azotemia; in advanced cirrhosis; no intrinsic renal disease

Causes and Pathogenesis

Splanchnic vasodilation causes renal hypoperfusion and functional failure.

Treatment

Albumin + terlipressin/norepinephrine; avoid nephrotoxins; TIPS/transplant

Preventive Measures

Avoid nephrotoxins; treat SBP early; monitor renal function.
