Implementation case study of malaria monitoring information for rural malaria workers using mobile r

An international medical study has brought new findings and related findings are of important reference value to the Cope for Disease Prevention. To support the eradication of malaria, Cambodia has implemented a mobile digital monitoring system based on local research, national integration, community-based real-time, case-based detection, reporting and response. This implementation case study describes the development, national deployment and operational performance of the Android-based mobile application VMAP.

These VMAP and MMAP are integrated into the national malaria information system MIS. The system was developed in collaboration with VMWs, which allows for real-time, geolocated case reporting, with online functionality, built-in verification checks, centralized device management, and automated analysis. The evaluation was conducted using three data sources: Regular Monitoring Data (2018-2024); VMW/MMWs in three high-risk provinces (2024); and National Technology User Interface Survey (2025); including reporting timeliness, completeness and monitoring performance indicators with the National Malaria Eradication Framework and the matching of user experience.

During the 2017-2018 national implementation phase, data integrity and timeliness were improved. During the 2018-2024 reporting period, reporting coverage increased from 19% to 99% and data integrity has remained above 99% since 2019; the 24-hour case notification rate increased from 2% to 99%, and the seven-day case observation rate increased from 0% to 98%; malaria pathogen detection rates increased from 135,664 to 715,277 annually. Enhanced monitoring and active malaria pathogen concentrations increased from 129 to 0 in 2024.

Out of 88 VMWs, 88 showed strong support for enhanced monitoring activities, including reporting and data analysis at the community level. A technical assessment survey found that 94% of VMWs were very satisfied with the application/satisfied with the application, experienced technical problems and never/rarely experienced 82% of VMWs' critical operational challenges including intermittent internet connectivity, limited electricity supply, and transportation restrictions, although offline functionality and data simultaneously prevented data loss. Cambodia's experience demonstrates the feasibility and sustainability of large-scale digital monitoring in a resource-limited environment, and emphasizes the importance of integration, training and sustainability through local ownership.
